To help you get the maximum benefit from your insurance plan, we want you to be aware of the following:

Some insurance plans require prior authorization so it is essential that you give us accurate insurance information before or at the time of your visit. Inaccurate information results in insurance companies rejecting claims and increased out of pocket expenses for you. There are some insurance companies that have more than one plan and not all providers are covered.
